If you have made an appointment with us, we request that you complete our patient questionnaire information AT LEAST 24 HOURS IN ADVANCE of your appointment so that we may provide you with excellent care when you arrive!  Filling out the necessary information beforehand will allow us to manage your valuable time wisely, and make your visit a simple one!

Our forms synchronize with our practice software to minimize both processing time AND additional burden on the environment (no printing is necessary!).   We appreciate your cooperation.  If you have any questions...please do not hesitate to call us at 503-644-5433.

1)  DO NOT hit the "submit" button until you have reached the very last page of each form. 

2)  Once you begin, please complete the applicable forms within 30 minutes or your data will not transfer

3)  Our system provides safe encryption of your information...that only our office is allowed to see.

4)  Do not worry about signing the forms.  We will obtain your electronic signature when you arrive at the office.
